MHS Sack Lunches Parent Order

Sack lunches are available for students to take on field trips.  
The Sack Lunch includes:

Sub Sandwich w/ Turkey Ham & Cheese
or Cheese Sandwich
Dragon Punch 100% Juice
Baby Carrots & Dip
Fresh Fruit 
Choice of Milk: White or Chocolate
Graham Treat

Cost of the Sack Lunch is based on each student’s eligibility just as lunches served at school.
 Paid - $2.30                Reduced - $.40                Free - $0.00
Your student’s account will be charged for a sack lunch or you can send payment to your child’s teacher.  The lunch will be ready & waiting on the morning of the trip!

MHS Limited Excused Absence

Complete the MHS Limited Excused Absence form and return to the Office of Students Services two days prior to planned absences. Teachers have the prerogative to require students to turn in assignments before leaving.

MHS Parent Teacher Conference Request

First semester parent-teacher conferences are scheduled for Thursday, October 20, and Friday, October 21, 2015.  (Note that there will be no school on either day.)

In order that we may efficiently schedule the conferences, please fill out the Parent-Teacher Conference form and return it to Amy Zimmerman in the Morton High School main office no later than October 17. You may drop it off, mail it (address is Morton High School, 350 N. Illinois Avenue, Morton, IL  61550.)  or send email  [email protected].   Your conference times will then be confirmed with the information mailed back to you.

Transcript Amendment Request Form

To the Parents/Guardians of grade 11 and 12 students:

Each high school junior takes the SAT in April.  The SAT is one measure of the progress your child has made toward college readiness. 

Please sign below indicating whether you do or do not want all of your student’s SAT, ACT, and Advanced Placement Exam scores affixed to the student’s transcript.  (Please note a single score cannot be sent alone or deleted.)  Please complete the notice below and return the signed document to the Counseling Office by March 29th.  If you fail to return the form, your child’s SAT, ACT, and Advanced Placement Exams will NOT be affixed to the transcript.
